All Customers seeking to receive a rebate must have “Pre” and “Post” Home Energy Assessment performed prior to filling out the rebate application form. To schedule a Home Energy Assessment, please contact the HELPS hotline at (888) 333-7525 to schedule a Home Energy Assessment.
Once work and the post audit are completed, please fill out the rebate application form below, being sure to submit a paid receipt and invoice.
